are required prior to receiving the Sacraments of First Holy Communion and Confirmation.
Generally, First Holy Communion is received at end of year 2 of program, corresponding to 2nd grade.
NOTE: If your child has not completed his/her first year of Religious Formation, please contact the Faith Formation office to discuss and plan accordingly
To best prepare for Confirmation, families are encouraged to remain enrolled in the Faith Formation Program following First Holy Communion.
At minimum, candidates must enroll in the 7th year program in order to receive the Sacrament by the end of the year 2 of Confirmation preparation, corresponding with completion of 8th grade.
NOTE: If your child has not received his/her first-year of Religious Formation or has not been enrolled in any Faith Formation Program since receiving First Holy Communion, please contact the Faith Formation office

Please download and complete the above form to register your child(ren) for Faith Formation classes starting in September 2026. Fees for this year can be found in the table below.
NOTE: LATE FEE of $25 will be charged for any registrations received AFTER SEPTEMBER 13, 2026.
PAYMENT:
1. ONLINE GIVING (credit card/ACH). Click link below, follow prompts and select 09-01 Faith Formation Fee in the dropdown menu, typing "Faith Formation Fee 2026-27" in the Note section.
2. PERSONAL CHECK: All check payments are to be payable to SAINT ANN CATHOLIC CHURCH.
Please note the following:
If you are using a physical check, please email the completed Registration Form to [email protected] and indicate that your payment will be either 1. sent via US Mail or 2. brought to the Parish Office.

FAITH FORMATION FEES 2026-2027
** ATTENTION **
The Faith Formation fees for Children in non-sacramental Religious Education, grades 3 thru 6, please refer to FEE TABLE below.
TWO-YEAR SACRAMENTAL PREP STUDENTS for FIRST COMMUNION (1st Grade) and CONFIRMATION (7th Grade)
Families enrolling their children for Sacramental Preparation will pay a non-refundable fee of $225.oo for two years of Faith Formation instruction. If the children have sibling(s) in non-sacramental years, the Faith Formation Fee for those children will be $50 per child. -

NOTE: Sacramental Preparation fees are NON-REFUNDABLE.
Non-Sacramental Faith Formation fees are refundable within SIXTY (60) days of payment.
LATE FEE of $25 will be charged for any registrations beginning MONDAY SEPTEMBER 14, 2026. -
